Both medical providers as well as healthcare team members involved in care can refer a child using our online form.
The purpose of Make-A-Wish is to grant the wish of each child older than 2½ years and under 18 years diagnosed with a critical illness currently placing the child’s life in jeopardy.
Please review our Referral Guidance Sheets (see below listing), which describe the medical conditions that typically qualify a child for a wish. There may be medical conditions not described on these sheets which can be considered on a case-by-case basis to determine if criteria is met for eligibility.

HIPAA Compliance
The Make-A-Wish Foundation ® makes every effort to assist our healthcare partners in remaining compliant with HIPAA guidelines. Prior to making contact with the healthcare team, Make-A-Wish works to ensure the family is aware of the inquiry and has agreed to voluntarily be a part of the referral process. The new online Wish & Volunteer Center helps to further ensure privacy by providing a secure, password protected link for the healthcare team to enter medical information. This password is computer generated and is unique to each referral entered.
Encryption Capabilities
Salesforce has many customers that are subject to laws pertaining to the processing of personally identifiable information (PII) or personal data. Salesforce offers its customers a broad spectrum of functionalities and customer-controlled security features that its customers may implement in their respective uses of the Salesforce services. Salesforce believes that these provide its customers the flexibility to comply with laws with stringent privacy and security requirements.
All transmissions between the user and the Salesforce Services are TLS encrypted with a 2048-bit Public Key. The Services use International/Global Step Up TLS certificates, with AES 256-bit encryption by default.
Salesforce takes the safety of its clients' data very seriously. In fact, it has been certified to comply with a number of international standards, including PCI DSS, FISMA, ISO/IEC 27001:2005, SAS 70 Type II, SysTrust, and Eu-US and Swiss-US Safe Harbor.
